Note on entry: At the beginning, an individual insurance policy number is requested: Please select the option "No insurance policy number received" below to start with the claim notification. In the further course, the following data should then be entered in the "Supplementary information" field:

  1. the name of the card-issuing bank = M.M.Warburg & CO
  2. the first six and last four digits of the credit card number (example: 123456######1234)
